You are required to demonstrate immunity to vaccine preventable diseases before you undertake clinical placements in a hospital or health service.  You will not be able to attend your placement until your immunisation record has been verified and approved by the UQ Immunisation Record Team.

To demonstrate your immunity, you are required to submit a Student Immunisation Record that has been completed by a registered General Medical Practitioner (GP).  The type of record you will need to complete will depend on the program you are completing.  For example, Doctor of Medicine students need to complete Student Immunisation Record (Form B).

Your completed Student Immunisation Record should be submitted via your my.UQ portal.  Commencing Year 1 MD students will receive a request to upload these documents in January.  All other students and updates will receive a new request, as required.

See below for further information on what you need to do, and how to submit your documentation:

Student Immunisation Record (Form B): Dentistry, Midwifery and Medicine and TB Self-Assessment Form

Student Immunisation Record (Form A):  Allied Health

Doctor of Medicine students are also required to provide COVID-19 vaccination evidence.  Please see further information below.
