Before commencing clinical placements at a hospital or health service, it is mandatory to provide evidence of immunity to vaccine-preventable diseases. Participation in your placement will only be permitted once l your immunisation record has been verified and approved by the UQ Immunisation Record Team.

To demonstrate your immunity, you are required to submit a Student Immunisation Record that has been completed by a registered General Medical Practitioner (GP).  The specific form  you will need to complete will depend on the program you are completing.  For example, Doctor of Medicine students need to complete Student Immunisation Record (Form B).

Your completed Student Immunisation Record should be submitted via your my.UQ portal.  Commencing Year 1 MD students will receive a request to upload these documents in January.  All other students and updates will receive a new request, as required.
